What is Breast Implant Removal?

Breast implant removal, or explant surgery, is a cosmetic procedure that removes silicone or saline breast implants. During implant removal surgery, our surgeons can replace implants with new ones or your own body fat. In some cases, they may reshape the breasts and allow them to heal without implant replacement. Breast lifts are often combined with breast implant removal procedures or replacement surgery to restore a more youthful shape and position of your breasts.

Capsular contracture occurs when scar tissue tightens around the implant. This can cause pain, discomfort, and tightness in the chest. It can also make the implants look and feel deformed. These breast issues are commonly treated with an en-bloc capsulectomy. Our board-certified specialists are experts in performing en-bloc capsulectomy.

A rupture occurs when the breast implant’s outer shell rips or tears, which may cause the inner contents to leak. Breast implant rupture can be caused by normal aging of the implant, trauma to the breast, a needle insertion during a biopsy, or other factors. When saline or silicone gel-filled implants rupture, the implants should be surgically removed or replaced.

Scar tissue, implant rippling, and weakened breast skin can cause visible deformities. Rippling occurs when the implant develops wrinkles and folds, which may be visible through the skin. These breast anomalies can affect the size, shape, and contours of your breasts. Our surgeons can address these concerns by removing or replacing the implants.

Breast implant illnesses (BII) are adverse physical and mental symptoms that a very small percentage of patients may experience after breast augmentation with implants. Although BII is a relatively new breast-related issue, initial research indicates breast implant removal often corrects BII symptoms.

Total Capsulectomy

During a total capsulectomy, doctors remove the entire breast capsule scar tissue around the breast implant. The procedure includes removing the front and back wall of the implant capsule and the implant itself.

Partial Capsulectomy

During a partial capsulectomy, surgeons only remove a portion of the scar tissue. This procedure can soften the implants to improve the overall appearance of your breasts.

En Bloc Capsulectomy

During an en bloc (“all together”) capsulectomy, plastic surgeons remove the entire capsule of scar tissue and the implant in one piece. This procedure is commonly performed for patients with breast implant illness (BII) symptoms.

Our surgeons can also use this technique to help patients with Breast Implant-Associated Anaplastic Large Cell Lymphoma (BIA-ALCL) symptoms. BIA-ALCL is a rare implant-related cancer that can cause pain, swelling, lumps in and around the breast, and fluid buildup. Symptoms typically begin to appear between three to fourteen years after implant placement. 

BIA-ALCL is an uncommon but potentially life-threatening condition. If you suspect you may have symptoms of BIA-ALCL, we urge you to schedule a consultation with one of the Pacific Center for Plastic Surgery surgeons as soon as possible to ensure you receive the best care quickly. 

After your surgery in our Newport Beach facility, you will usually need some time to heal and recover. During this time, your breasts will likely be swollen and tender. Depending on your surgery, your surgeon may insert drains to allow proper healing and prevent infection. You may also need to wear a compression bra to reduce swelling.

While the exact recovery timeline is different for each person, many patients feel like they are “back to normal” around six to eight weeks after surgery. Combining breast implant removal with additional procedures like fat transfer or a breast lift may increase your recovery time.

Before surgery, your surgeon will provide detailed instructions about any recovery restrictions. They will also share the best practices to follow to ensure a healthy recovery and optimal results.

Your complete results will depend on the condition of your breasts and the goals of your procedure. It may take up to a year for some patients to see the final results as swelling decreases and the breasts settle with time. 

If you opt for new implants, you can expect them to last ten years or more. However, we recommend you replace your implants eight to ten years after surgery to prevent rupture or other cosmetic concerns.  

Patients may choose this surgery for cosmetic or medical reasons. Ideal candidates for breast implant removal surgery may have the following concerns: 

  • Your breast implants are causing pain or discomfort.
  • Your breast implants feel too heavy or get in the way of normal activities.
  • You can feel your breast scar tissue getting harder or tighter.
  • Your breast implants don’t look or feel right, and you want an aesthetic change.

Ideal candidates are also physically healthy and maintain a stable weight. Depending on your reason for removing the breast implants, there are situations where you may not be a good candidate for breast implant removal. After breast augmentation, you will usually have to wait until your breasts have fully healed before we can accurately assess your condition.
