What is En Bloc Breast Implant Explantation?

En Bloc breast implant explantation is a type of breast implant removal procedure. During a traditional breast implant removal, the capsule is opened and the implant is removed. However, during En Bloc breast implant removal, the entire capsule with the implant inside is removed. The capsule refers to the scar that the body forms around a foreign object, in this case, the implant.

Who Needs En Bloc Breast Implant Explantation?

En Bloc breast implant explantation might be recommended for the following situations where the entire capsule needs to be removed:

Capsular Contracture

The forming of a capsule is a normal part of the healing process after breast augmentation. A regular capsule will not be felt. However, sometimes the capsule thickens and tightens, resulting in a hardening of the implant known as capsular contracture. When this happened, En Bloc breast implant removal can be used to remove the entire capsule from the body, replacing the implant with a fresh one.

Permanent Removal

En Bloc breast implant removal might also be performed when a patient has their implants taken out and will not be having them replaced. In these cases, the capsule is not needed and can be taken out with the implant intact. Permanent removal may be necessary if the patient experiences breast implant illness, more often associated with silicone implants. While very rare, some individuals have shown to have a reaction to silicone implants with symptoms such as fatigue, headache, joint pain, hair loss, and rashes. If these symptoms occur, En Bloc breast implant removal will ensure that the foreign object is completely removed.

Silicone Implant Removal

En Bloc breast implant explantation can be used for the above reasons on either silicone or saline implants. Saline implants are composed of sterile salt water that can be safely absorbed by the body. Still, if a capsular contracture forms around a saline implant, this will require En Bloc explantation to ensure that all scar tissue is removed. The procedure becomes more important when dealing with silicone implant removal. Silicone implants are composed of gel that can be harmful if leaked into the surrounding tissue. Oftentimes with older implants, the implant will leak but remain inside the capsule, preventing it from entering the body and causing harm. In these cases, we want to ensure that the silicone is completely removed without allowing any gel to enter the body. During En Bloc removal, the entire capsule is removed to ensure that the entire implant and any possible leaking gel is completely taken out.

En Bloc Breast Implant Explantation Process

During En Bloc breast implant explantation, a small incision is made and the entire capsule is gently dissected away from the surrounding tissue. Patients may choose to have the implant removed completely. In this case, a breast lift might be performed after removal to address any loose skin and return the breasts to their natural position. If the implants are being replaced, a fresh new implant will be reinserted into the breast pocket.

En Bloc Breast Implant Explantation Results

After En Bloc breast implant removal, patients will have 1-2 weeks of recovery, similar to the original breast augmentation procedure. Some swelling, bruising, and general discomfort can be expected. Once healed, patients can enjoy relieved symptoms from breast implant illness or capsular contracture. New implants can last for many more years.
